*Job Summary:*

We are seeking a dedicated and compassionate Geriatric Care Nurse to join our client's team in Berlin. The successful candidate will provide specialized nursing care to elderly patients, focusing on enhancing their quality of life, managing chronic conditions, and promoting independence and well-being.

*Key Responsibilities:*

- Assess the health status of elderly patients and develop individualized care plans.
- Administer medications and treatments as prescribed by healthcare providers.
- Monitor patient progress and adjust care plans as needed.
- Educate patients and their families on managing chronic conditions, medications, and lifestyle changes to improve health outcomes.
- Collaborate with multidisciplinary teams to ensure comprehensive care for patients.
- Maintain accurate and up-to-date patient records.
- Provide emotional support and compassionate care to patients and their families.

*Qualifications:*

- Bachelor’s degree in Nursing or equivalent.
- Valid nursing license in Germany.
- Experience in geriatric nursing or a related field.
- Strong interpersonal and communication skills, particularly in German.
- Ability to work independently and as part of a multidisciplinary team.
- Empathetic and patient-focused approach.
